Be It Ordained by the City Council ofthe City of Chicago:
SECTION 1. Permission and authority are hereby given and granted to NORTHWESTERN MEMORIAL HOSPITAL, upon the terms and subject to the conditions of this ordinance to maintain and use, as now constructed, nine (9) Valve(s) under the public right-of-way adjacent to its premises known as 245 E. Chicago Ave Said sewer lines represent the connection of various waste lines from the new Prentice Women's Hospital to the main sewer on East Superior Street. Sewer lines shall be described as follows:
Said Valve(s) at E. SUPERIOR measure(s):
One (1) at thirty (30) feet in length, and point eight three (.83) feet in width for a total of twenty-four point nine (24.9) square feet.
One (1) at six (6) feet in length, and point eight three (.83) feet in width for a total of four point nine eight (4.98) square feet.
One (1) at eight (8) feet in length, and point eight three (.83) feet in width for a total of six point six four (6.64) square feet.
One (1) at five (5) feet in length, and point eight three (.83) feet in width for a total of four point one five (4.15) square feet.
One (1) at sixteen (16) feet in length, and point six seven (.67) feet in width for a total of ten point seven two (10.72) square feet.
Said Valve(s) at N. Fairbanks Court measure(s): f
One (1) at eight (8) feet in length, and one (1) foot in width for a total of eight (8) square feet.
One (1) at thirty (30) feet in length, and point eight three (.83) feet in width for a total of twenty-four point nine (24.9)
square feet.
Said Valve(s) at E. Chicago Avenue measure(s):
One (1) at five (5) feet in length, and one (1) foot in width for a total of five (5) square feet.
One (1) at forty (40) feet in length, and point eight three (.83) feet in width for a total of thirty-three point two (33.2)
square feet.
The location of said privilege shall be as shown on prints kept on file with the Department of Business Affairs and Consumer Protection and the Office of the City Clerk.'
Said privilege shall be constructed in accordance with plans and specifications approved by the Department of Transportation (Office of Underground Coordination) and Department of Water Management.
This grant of privilege in the public way shall be subject to the provisions of Section 10-28-015 and all other required provisions of the Municipal Code of Chicago.
The grantee shall pay to the City of Chicago as compensation for the privilege #1096758 herein granted the sum of three thousand six hundred ($3,600.00) per annum in advance.

A 25% penalty will be added for payments received after due date.
The permit holder agrees to hold the City of Chicago harmless for any damage, relocation or replacement costs associated with damage, relocation or removal of private property caused by the City performing work in the public way.
Authority herein given and granted for a period of five (5) years from and after 05/24/2011.
