ADVOCATE ILLINOIS MASONIC MEDICAL CENTER Acct. No. 63145 - 2 Permit No. 1123994
Be It Ordained by the City Council of the City of Chicago:
SECTION 1. Permission and authority are hereby given and granted to ADVOCATE ILLINOIS MASONIC MEDICAL CENTER, upon the terms and subject to the conditions of this ordinance to maintain and use, as now constructed, one (1) three (3) inch steam pipe in an eight (8) inch casing, one and one-half (11/2) inch condensate pipe in a five (5) inch casing, two (2) six (6) inch chilled water pipes in an eleven (11) inch insulated casing, crossing under and across West Wellington Avenue approximately one hundred ninety (190) feet west of the centerline of North Mildred Avenue adjacent to its premises known as 836 West Wellington Avenue. Also, two (2) six (6) inch concrete pipes buried below the frost line across West Nelson Street approximately fifteen (15) feet west of North Dayton Street at a depth of approximately four (4) feet. One (1) pipe contain three (3) one and one-half (1 V*) inch conduits for power and alarm telephone lines. The pother pipe contain an oxygen line.
The location of said privilege shall be as shown on prints kept on file with the Department of Business Affairs and Consumer Protection and the Office of the City Clerk.
Said privilege shall be constructed in accordance with plans and specifications approved by the Department of Transportation (Office of Underground Coordination) and Department of Water Management.
This grant of privilege in the public way shall be subject to the provisions of Section 10-28-015 and all other required provisions of the Municipal Code of Chicago.
The grantee shall pay to the City of Chicago as compensation for the privilege #1123994 herein granted the sum of eight hundred ($800.00) per annum in advance.
A 25% penalty will be added for payments received after due date.
