For the erection of 1 (four-sided) sign over 24 feet above ground and over 100 square feet at:

1801 W. Jackson Blvd. Chicago, IL 60612

Dimensions: length 15'-0"   height 31'-6"    width 5'-0"

Height above grade/roof to top of sign:    31 '-6"

Total Square Foot Area (front and side counted):      630 Square Feet

Such sign shall comply with all applicable provisions of TITLE 17 of the Chicago Zoning Ordinance and all other applicable provisions of the Municipal Code of the City of Chicago governing the construction and maintenance of outdoor signs, signboards and structures.
