The ordinance passed by the City Council of the City of Chicago for FRESENIUS MEDICAL CARE VENTURES, LLC on 07/24/2013, and printed upon page 58129 of the Journal of Proceedings of the City of Chicago is here by Amended by deleting the words "2734 N Milwaukee Ave: Said sign structure is located along N. Milwaukee Avenue" and inserting in their place the words "2721 N Spaulding Ave: Said sign structures is located along N. Spaulding Ave"
 
 
Section 2:
 
This ordinance Amendment shall be in effect upon its passage.

The applicant referenced above has requested the use of the public right-of-way for a sign(s). An ordinance has been prepared by the Department of Business Affairs and Consumer Protection - Business Assistance Center - Public Way Use Unit for presentation to the City Council. Because this request was made for properties located in your ward, as approved by you as per the attached, I respectfully request that you introduce the attached ordinance at the next City Council meeting.
