NORTHWESTERN MEMORIAL HOSPITAL Acct. No. 85392 - 6 Permit No. 1119864

Be It Ordained by the City Council ofthe City of Chicago:
SECTION 1. Permission and authority are hereby given and granted to NORTHWESTERN MEMORIAL HOSPITAL, upon the terms and subject to the conditions of this ordinance to maintain and use, as now constructed, four (4) Sanitary Sewer Main(s) under the public right-of-way adjacent to its premises known as 221 E. Huron St
Said Sanitary Sewer Main(s) at E. Huron Street measure(s):
One (1) at ten (10) feet in length, and point eight three (.83) feet in width for a total of eight point three (8.3) square feet. Existing sanitary sewer is approximately eight (8) feet below grade level.
Said Sanitary Sewer Main(s) at N. Fairbank Street measure(s):
One (1) at twenty-four (24) feet in length, and point eight three (.83) feet in width for a total of nineteen point nine two (19.92) square feet. Existing sanitary sewer is approximately eight (8) feet below grade level.
Said Sanitary Sewer Main(s) at E. Erie Street measure(s):
One (1) at ten (10) feet in length, and point eight three (.83) feet in width for a total of eight point three (8.3) square feet. Existing sanitary sewer is approximately eight (8) feet below grade level.
Said Sanitary Sewer Main(s) at N. St Clair Street measure(s):
One (1) at twenty-four (24) feet in length, and point eight three (.83) feet in width for a total of nineteen point nine two (19.92) square feet. Existing sanitary sewer is approximately eight (8) feet below grade level.
The location of said privilege shall be as shown on prints kept on file with the Department of Business Affairs and Consumer Protection and the Office of the City Clerk.
Said privilege shall be constructed in accordance with plans and specifications approved by the Department of Transportation (Office of Underground Coordination) and Department of Water Management.
