RESOLUTION CELEBRATING 100tn BIRTHDAY OF HAZEL AMELIA COOK (NEE MORNER)
WHEREAS, Hazel Amelia Cook celebrates her 100th birthday on May 19, 2016; and
WHEREAS, the Chicago City Council has been informed of this momentous event by Alderman Debra L. Silverstein; and
WHEREAS, Hazel Amelia Cook was born in Minneapolis, Minnesota on May 19, 1916 to Gustav and Cecilia Morner, and is one of three siblings. She moved to Chicago in 1926 and graduated from Lake View High School in 1934; and
WHEREAS, Hazel Amelia Cook married John L. Cook on December 24, 1938 at Bethany Evangelical Lutheran Church. They purchased a two-flat in West Ridge, and she has continued to live in West Ridge since that date. Together, they raised two children, Carol Ann and Linda Jean (Donald) Zaloudek; and
WHEREAS, Hazel Amelia Cook is the loving grandmother of Zoe Ann Zaloudek, who is married to Brad McVay; and
WHEREAS, Hazel Amelia Cook is a devoted mother, grandmother and cherished friend to many, and richly deserves the warm and fond wishes of her family and friends as she marks her birthday celebration; now, therefore,

BE IT RESOLVED, that we, the Mayor and members of the City Council of the City of Chicago, assembled this thirteenth day of April, 2016, do hereby express our heartiest congratulations to Hazel Amelia Cook in celebration of her 100 birthday, and extend to her our very best and most sincere wishes for continued happiness and prosperity.
