ORDINANCE
BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF CHICAGO:
SECTION l. Title 17 ofthe Municipal Code of Chicago, the Chicago Zoning Ordinance, is hereby amended by changing all of the RS3 Residential Single-Unit (Detached House) District symbols and indications as shown on Map No. 7-J in the area bounded by
A line 75 feet north of and parallel to West Barry Avenue; North Kimball Avenue; a line 25 feet north of and parallel to west Barry Avenue; the public alley next west of and parallel to North Kimball Avenue,
to those of an RT4 Residential Two-Flat, Townhouse and Multi-Unit District.
SECTION 2. This ordinance shall be in force and effect from and after its passage and due publication.

The undersigned, Lisa Perna Miller, being first sworn on oath deposes and states the following:
The undersigned certifies that he has complied with the requirements of Section 17-13-0107 ofthe Chicago Zoning Ordinance, by sending written notice to such property owners who appear to be the owners ofthe property within the subject area not solely owned by the applicant, and to the owners of all property within 250 feet in each direction ofthe lot line ofthe subject property, exclusive of public roads, streets, alleys and other public ways, or a total distance limited to 400 feet. Said "written notice" was sent by First Class U.S. Mail, no more than 30 days before filing the application.
The undersigned certifies that the notice contained the address ofthe property sought to be rezoned; a statement ofthe intended use of the property; the name and address ofthe applicant; the name and address ofthe owner; and a statement that the applicant intends to file the application for a change in zoning on approximately October 31, 2014.
