Title: Recognition of Community Vaccine Alliance for vaccination efforts

RESOLUTION HONORING COMMUNITY VACCINE ALLIANCE

WHEREAS, Community Vaccine Alliance was formed to address the need of providing fair and equitable vaccine access to community members on Chicago's Northwest side. In partnership with local government, Agudath Israel of Illinois, Hatzalah Chicago, and Refuah311 are member organizations of Community Vaccine Alliance; and

WHEREAS, During the COVID-19 pandemic, the 60659 zip code and Pulaski Park and West Ridge neighborhoods had some ofthe highest rates of COVID-19 infection in the City of Chicago; and '

WHEREAS, Community Vaccine Alliance hosted over a dozen clinics to vaccinate survivors ofthe Holocaust, the elderly, front line workers, first responders, educators, care givers, and all eligible members ofthe community; and

WHEREAS, Over 500 volunteers came together under the leadership of Community Vaccine Alliance; and

WHEREAS, Nearly 7,000 residents were fully vaccinated, aiding the City of Chicago in immunizing communities most affected by COVID-19; and

BE IT RESOLVED, That we, the Mayor and Members ofthe City Council of Chicago, assembled on this twenty-sixth day of May, 2021 do hereby honor Community Vaccine Alliance for their service and the service of its volunteers in helping vaccinate thousands of members of the community and mitigating the spread of COVID-19; and

BE IT FURTHER RESOLVED, That suitable copies ofthis resolution be presented to Community Vaccine Alliance in gratitude and recognition for these contributions to our great
