At the request of the Commissioner of Fleet and Facility Management, I transmit herewith ordinances authorizing the execution of lease agreements.

ORDINANCE BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF CHICAGO:
SECTION 1: On behalf of the City of Chicago as Landlord, the Commissioner of the Department of Fleet and Facility Management and the Commissioner of the Department of Family and Support Services are authorized to execute a Lease with Franciscan Outreach Association, as Tenant, governing the use of approximately 1,400 square feet of space located at 10 South Kedzie Avenue; such Lease to be approved as to form and legality by the Corporation Counsel in substantially the following form:

THIS LEASE is made and entered into this day of , 2013 (the
"Commencement Date"), by and between THE CITY OF CHICAGO, an Illinois Municipal Corporation and Home Rule Unit of Government (herein referred to as "Landlord" or "City") and FRANCISCAN OUTREACH ASSOCIATION, an Illinois Not-for-Profit Corporation (hereinafter referred to as "Tenant").
RECITALS
WHEREAS, Landlord is the owner of the building located at 10 South Kedzie Avenue and comprised of approximately 62,660 square feet more commonly known as the Garfield Park Community Center, located at 10 South Kedzie Avenue, Chicago, Illinois, as legally described in Exhibit A attached hereto and made a part hereof ( the "Building"); and
WHEREAS, Landlord's Department of Family and Support Services has selected Tenant to perform certain services as outlined in the Delegate Agency Grant Agreement attached hereto and made a part hereof as Exhibit B; and
