ORDINANCE
ACADIA CLARK-DIVERSEY Acct. No. 379011 -3 Permit No. 1126149

Be It Ordained by the City Council of the City of Chicago:
SECTION 1. Permission and authority are hereby given and granted to ACADIA CLARK-DIVERSEY, upon the terms and subject to the conditions of this ordinance to construct, install, maintain and use three (3) Earth Retention System(s) under the public right-of-way adjacent to its premises known as 2747 N. Clark St
Said Earth Retention System(s) at N. Clark Street measure(s):
One (1) at twenty-one (21) feet in length, and one (1) foot in width for a total of twenty-one (21) square feet. Said earth retention system shall be approximately nineteen (19) feet below grade level.

Said Earth Retention System(s) at W. Diversey Pkwy measure(s):
One (1) at ninety-three (93) feet in length, and one (1) foot in width for a total of ninety-three (93) square feet. Said earth retention system shall be approximately nineteen (19) feet below grade level.
Said Earth Retention System(s) at Public Alley measure(s):
One (1) at fifteen (15) feet in length, and one (1) foot in width for a total of fifteen (15) square feet. Said earth retention system shall be approximately nineteen (19) feet below grade level.

The location of said privilege shall be as shown on prints kept on file with the Department of Business Affairs and Consumer Protection and the Office of the City Clerk.
Said privilege shall be constructed in accordance with plans and specifications approved by the Department of Transportation (Office of Underground Coordination).
This grant of privilege in the public way shall be subject to the provisions of Section 10-28-015 and all other required provisions of the Municipal Code of Chicago.
The grantee shall pay to the City of Chicago as compensation for the privilege #1126149 herein granted the sum of one thousand two hundred ($1,200.00) per annum in advance.
A 25% penalty will be added for payments received after due date.
