NEW CINGULAR WIRELESS PCS, LLC i' Acct. No. 89154-40 Permit No. 1116103
Be It Ordained by the City Council ofthe City of Chicago:
SECTION 1. Permission and authority are hereby given and granted to NEW CINGULAR WIRELESS PCS, LLC, upon the terms and subject to the conditions of this ordinance to construct, install, maintain and use three (3) Exterior mounts (Antennas) projecting over the public right-of-way adjacent to its premises known as 118 E. Erie St
Said installation and operation of antennas and associated equipment for AT&T's wireless telecommunications network. Installation of two (2) wall mounted WI-FI antennas, six (6) stealth canisters, one (1) RTP antenna, six (6) access points and four (4) triplexers.
Said Exterior mounts (Antennas) at N. Michigan Avenue measure(s):
Two (2) at nine point four one (9.41) feet in height and four point six four (4.64) feet in depth. Said antennas shall be approximately forty-nine (49) feet above grade level.
One (1) at fourteen point five (14.5) feet in height and three point eight (3.8) feet in depth. Said antenna shall be approximately one hundred sixteen (116) feet three (3) inches above grade level.
The location of said privilege shall be as shown on prints kept on file with the Department of Business Affairs and Consumer Protection and the Office of the City Clerk.
Said privilege shall be constructed in accordance with plans and specifications approved by the Department of Transportation (Division of Project Development).
This grant of privilege in the public way shall be subject to the provisions of Section 10-28-015 and all other required provisions of the Municipal Code of Chicago.
The grantee shall pay to the City of Chicago as compensation for the privilege #1116103 herein granted the sum of one thousand two hundred ($1,200.00) per annum in advance.
A 25% penalty will be added for payments received after due date.
