WHEREAS, Janics J Kozak "Koz" has been called to eternal life by the wisdom of God to his

WHEREAS.The Chicago Giry Council has beeninformed of his passing by the Honorable Patrick D. Thompson, Aldcmian of the 11 th Ward; and
WHEREAS, James J Kozak, Long time partner to Lisa Martinclli; beloved son of the late James and Marilyn Kozak; second,son to Lizzy Martinelli; dear brother of Kathleen ( James) - • ; Case. Marlene (Lawrence) Koziol, Michael (Susan) Kozak, Edward (Erin) Kozak arid Mary (Ronald) Bzdyl; fond nephew, cousin, uncle, and friend to many; and
WHEREAS, His love of life and ability to live it to the fullest endearedfjames to his family
members, friends and all who knew him, and enabled him to enrich their lives in ways they will
never forget; now. therefore •, '
