BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF CHICAGO:

SECTION 1: The Commissioner of the Department of Fleet and Facility Management is authorized to execute on behalf of the City of Chicago, as Landlord, a Lease with Near North Health Services Corporation, as Tenant, for Near North Health Services Corporation's use of a City-owned parking lot of approximately 13,250 square feet and located at 1014-24 East 43rd Street for non-public parking; such Lease to be approved as to form and legality by the Corporation Counsel in substantially the following form:

LEASE

THIS LEASE (the "Lease") is made and entered into this day of
, 2013 by and between, the CITY OF CHICAGO, an Illinois municipal
corporation and home rule unit of government (herein referred to as "City" or "Landlord"), and NEAR NORTH HEALTH SERVICE CORPORATION, an Illinois not-for-profit corporation (hereinafter referred to as "Tenant").

RECITALS

WHEREAS, Landlord is the owner of the parking lot located at 1014-24 East 43rd Street, Chicago, Cook County, Illinois as legally described on Exhibit A attached hereto; and

WHEREAS, the property located at 1014-24 East 43rd Street has no present municipal use; and

WHEREAS, Landlord has agreed to lease to Tenant, and Tenant has agreed to lease from Landlord, approximately 13,250 square feet of a paved lot located at 1014-24 East 43rd Street to be used by the Tenant as non-general parking only for Tenant's staff and patients to Tenant's Holman Neighborhood Health Center located at 4230 South Greenwood Avenue.
