Be It Ordained by the City Council of the City of Chicago:
SECTION 1. Permission and authority are hereby given and granted to Oak Street Health, upon the terms and subject to the conditions of this ordinance to construct, install, maintain and use four (4) Light Fixture(s) projecting over the public right-of-way adjacent to its premises known as 1541 W. Devon Ave
Said Light Fixture(s) at W Devon Ave measure(s):
Four (4) at point seven five (.75) feet in length, one point five (1.5) foot in width and sixteen (16) feet above grade £' , level.
The location of said privilege shall be as shown on prints kept on file with the Department of Business Affairs and Consumer Protection and the Office of the City Clerk.
Said privilege shall be constructed in accordance with plans and specifications approved by the Department of Transportation (Division of Project Development).
This grant of privilege in the public way shall be subject to the provisions of Section 10-28-015 and all other required provisions of the Municipal Code of Chicago.
The grantee shall pay to the City of Chicago as compensation for the privilege #1107367 herein granted the sum of ninety ($90.00) per annum in advance.
A 25% penalty will be added for payments received after due date.
The permit holder agrees to hold the City of Chicago harmless for any damage, relocation or replacement costs associated with damage, relocation or removal of private property caused bythe City performing work in the public way.
Authority herein given and granted for a period of five (5) years from and after Date of Passage.

The reflector is angled at 30* to projecl light backwards onto the wall where the sign would be mounted. It is made of aluminum (aluminum is standard; other materials are available), with stainless steel hardware. It accommodates the lamp socket, and possessos good thermal characteristics. The Sign 7 floodlight Is designed lo operate with LED (20W max.), compact fluorescent (4ZW max.), metal hairdo (70W max.), and Incandescent lamps (150W max.), for design And application flexibility. LED Driver

The exterior reflector and stem could be finished with a TMS standard colour using the same process, painted in a RAL colour, or plated wilh a TMS specialty finish, to comptcmont the theme. Mounting
The rigid, aluminum conduit is designed to be mounted perpendicular to a wan, whero the resulting 90* bend places the reflector in an Ideal position lo illuminate lall signage below -close to the sign, yet unobtrusive to view. Tho standard thread sire is 1/2" NPT, and 3/4" NPT is availablo for mounting in windy areas, or for special applications whero extra support is necessary.
Mount the Sign 7 floodlight on a wall, directly over a standard 4' electrical junction box wilh 3%' holes c/c. Tho wall mount ballast requires a singlo connection to line voltage, and Ihe remote mount ballast requires a connection to the lamp socket leads and another to the line voltago. Compliances
The Sign 7 is UL/CSA approved for indoor and outdoor locations, wet and dry (IP23). It complies with USTC standards and Consultants Europe directives.
