SECTION VII ACKNOWLEDGMENTS, CONTRACT INCORPORATION, COMPLIANCE, PENALTIES, DISCLOSURE
 
The Disclosing Party understands and agrees that:
  1. The certifications, disclosures, and acknowledgments contained in this EDS will become part of any contract or other agreement between the Applicant and the City in connection with the Matter, whether procurement, City assistance, or other City action, and are material inducements to the City's execution of any contract or taking other action with respect to the Matter. The Disclosing Party understands that it must comply with all statutes, ordinances, and regulations on which this EDS is based.
  2. The City's Governmental Ethics and Campaign Financing Ordinances, Chapters 2-156 and 2-164 of the Municipal Code, impose certain duties and obligations on persons or entities seeking City contracts, work, business, or transactions. The full text of these ordinances and a training program is available on line at www.cityofchicago.org/Ethics. and may also be obtained from the City's Board of Ethics, 740 N.

  1. If the City determines that any information provided in this EDS is false, incomplete or inaccurate, any contract or other agreement in connection with which it is submitted may be rescinded or be void or voidable, and the City may pursue any remedies under the contract or agreement (if not rescinded or void), at law, or in equity, including terminating the Disclosing Party's participation in the Matter and/or declining to allow the Disclosing Party to participate in other transactions with the City. Remedies at law for a false statement of material fact may include incarceration and an award to the City of treble damages.
  2. It is the City's policy to make this document available to the public on its Internet site and/or upon request. Some or all of the information provided on this EDS and any attachments to this EDS may be made available to the public on the Internet, in response to a Freedom of Information Act request, or otherwise. SECTION VII ACKNOWLEDGMENTS, CONTRACT INCORPORATION, COMPLIANCE, PENALTIES, DISCLOSURE

The certifications, disclosures, and acknowledgments contained in this EDS will become part of any contract or other agreement between the Applicant and the City in connection with the Matter, whether procurement, City assistance, or other City action, and are material inducements to the City's execution of any contract or taking other action with respect to the Matter.   2. The City's Governmental Ethics and Campaign Financing Ordinances, Chapters 2-156 and 2-164 of the Municipal Code, impose certain duties and obligations on persons or entities seeking City contracts, work, business, or transactions. • SUPPLEMENTAL SUBMISSIONS TYPE 1 REZONING Additional information for "Type 1" rezoning narrative:
The applicant, City Lights Church seeks to renovate the current dilapidated building and adjacent parking lots to use as its new church facility. The current facility was built as a banqueting hall and has been subsequently used as a food catering facility by the present owner. As a banqueting facility, the layout is easily accommodated for a church with a sanctuary, accessory Sunday school class rooms, and an area for fellowship. The planned renovation will neither increase the footprint nor the height of the building.
The property is situated on three lots, each with a different zoning classification which includes Cl-1 for the building; B3-1 for the adjacent parking lot; and Ml-1 for the lot exiting onto Narragansett Ave. Because the three lots comprise one zoning lot, the Ml-1 designation which does not allow for religious assemblies controls. Accordingly, the applicant seeks to rezone the 3 parcels to all be zoned as B3-1 so as to allow a church with accessory classrooms
The location ofthe property is in a buffer area between a Ml-1 zone to the West; a B3-1 and Ml-1 zone to the North across Grand Ave.; a B3-1 zone directly to the East; and a RS-3 zone to the South. As such the church is compatible with the neighborhood given that churches are considered compatible with residential areas, but due to parking and traffic, also fit well into a buffer commercial or business districts which are more likely to have adequate existing facilities and parking.
