APPLICATION FOR STATE-ISSUED AUTHORIZATION TO PROVIDE CABLE SERVICE
Pursuant to Section 401 of the Cable and Video Competition Law of 2007 ("the Law") (220 ILCS 5/21-100, et seq.), Comcast of Illinois III, Inc. d/b/a CHICAGO CABLE TV-IV ("Comcast") hereby applies for State-issued authority to use, occupy and construct facilities in the public right-of-way for the delivery of cable service and for State-issued authority to provide cable service in a geographic area located wholly within the City of Chicago, commonly identified as Chicago Area 4 and legally described in Attachment 1 hereto, and hereinafter referred to as the service area footprint ("service area footprint").
In support of this Application, Comcast submits the attached Affidavit of John Crowley, Senior Vice President of Comcast's Greater Chicago Region ("Affidavit"). The Affidavit contains the affirmations and information required by Section 401(b) ofthe Law (220 ILCS 5/21-401 (b)).
