BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF CHICAGO:


SECTION 1: On behalf of the City of Chicago, the Commissioner of the Department of Fleet and Facility Management is authorized to execute a Lease with the Westside Holistic Family Services Inc., governing the City's use of property located at 4909 West Division Street by the Department of Public Health, payment of rent, and indemnification; such Lease to be approved by the Commissioner of the Department of Public Health and approved as to form and legality by the Corporation Counsel in substantially the following form:

LEASE

THIS LEASE is made and entered into this day of , 2014 (the
"Effective Date"), by and between WESTSIDE HOLISTIC FAMILY SERVICES, INC., an Illinois Not-for-Profit Corporation (hereinafter referred to as "Landlord"), and THE CITY OF CHICAGO, an Illinois Municipal Corporation and Home Rule Unit of Government (hereinafter referred to as "Tenant" or "City").

RECITALS

WHEREAS, Landlord is the owner of a five story building more commonly known as 4909 West Division Street (the "Building") located in Chicago, Cook County. Illinois; and

WHEREAS, Landlord has agreed to lease to Tenant, and Tenant has agreed to lease from Landlord, Suite 412, Suite 411, and a portion of Suite 401 comprised of approximately 3,400 square feet of office space located on the fourth floor of the Building as described on Exhibit A attached hereto and made a part hereof and to be used by the Department of Public Health to administer Tenant's clinical and counseling programs.
