SUBSTITUTE ORDINANCE



WHEREAS, the Annual Appropriation Ordinance for the year 2015 of the City of Chicago (the "City") contains estimates of revenues receivable as grants from agencies of the state and federal governments and public and private agencies; and
WHEREAS, in accordance with Section 8 of the Annual Appropriation Ordinance, the heads of various departments and agencies of the City have applied to agencies of the state and federal governments and public and private agencies for grants to the City for various purposes; and

WHEREAS, the City through its Mayor's Office has been awarded private grant funds in the amount of $163,000 from the Rockefeller Foundation which shall be used for the 100 Resilient Cities Initiative; and
WHEREAS, the City through its Department of Public Health ("Health") has been awarded additional federal grant funds in the amount of $41,000 from the United States Department of Health and Human Services, Centers for Disease Control and Prevention ("CDC") which shall be used for the Bioterrorism Hospital Preparedness Program; and
WHEREAS, the City through Health has been awarded additional federal grant funds in the amount of $2,038,315 from CDC which shall be used for the Bioterrorism Preparedness Response Planning Program; and

WHEREAS, the City through Health has been awarded additional federal grant funds in the amount of $367,000 from CDC which shall be used for the HIV Behavioral Surveillance Program; and

WHEREAS, the City through Health has been awarded additional federal grant funds in the amount of $77,000 from CDC which shall be used for the Morbidity and Risk Behavior Surveillance Program; and

WHEREAS, the City through Health has been awarded federal grant funds in the amount of $1,000,000 from the United States Department of Health and Human Services which shall be used for the Teen Pregnancy Prevention Evaluation Program; and
