MCDONALD'S STORE #4061 Acct. No. 343287 -1 Permit No. 1120484

¦Be It Ordained by the City Council of the City of Chicago:
"SECTION 1. Permission and authority are hereby given and granted to McDONALD'S STORE #4061, upon the :. terms and subject to the conditions of this ordinance to maintain and use, as now constructed, two (2) Vault(s) under the public right-of-way adjacent to its premises known as 180-186 W. Adams St
Said Vault(s) at W. Adams Street measure(s):
One (1) at eighty-eight point six (88.6) feet in length, and ten point seven five (10.75) feet in width for a total of nine hundred fifty-two point four five (952.45) square feet. Existing vault is used for storage.

Said Vault(s) at S. Wells Street measure(s):
One (1) at forty-eight point two (48.2) feet in length, and thirteen point four (13.4) feet in width for a total of six hundred forty-five point eight eight (645.88) square feet. Existing vault is used for storage.
• The location of said privilege shall be as shown on prints kept on file with the Department of Business Affairs and Consumer Protection and the Office of the City Clerk.
Said privilege shall be constructed in accordance with plans and specifications approved by the Department of Transportation (Office of Underground Coordination).
This grant of privilege in the public way shall be subject to the provisions of Section 10-28-015 and all other required provisions of the Municipal Code of Chicago.
The grantee shall pay to the City of Chicago as compensation for the privilege #1120484 herein granted the sum of . ten thousand four hundred eighty-five ($10,485.00) per annum in advance.
A25% penalty will be added for payments received after due date.
