BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF CHICAGO:


SECTION 1: On behalf of the City of Chicago as Landlord, the Commissioner of the Department of Fleet and Facility Management is authorized to execute a Lease with True to Life Foundation, as Tenant, for use of a vacant City-owned property located at 8828 South Stony Island Avenue; such Lease to be approved by the Commissioner ofthe Department of Planning and Development, and approved as to form and legality by the Corporation Counsel in substantially the following form:

LEASE

THIS LEASE ("Lease"), is made and entered into this day of
, 2015 (the "Commencement Date) by and between, THE CITY OF
CHICAGO, an Illinois municipal corporation and home rule unit of government (hereinafter referred to as "Landlord"" or "City"), and TRUE TO LIFE FOUNDATION, an Illinois not-for-profit Corporation (hereinafter referred to as "Tenant").


RECITALS

WHEREAS, the Landlord is the owner of the former Chicago Public Library Avalon Branch, located at 8828 South Stony Island Avenue, Chicago, Cook County, Illinois, PINs 25-02-215-028, -029, -030, -031 (the "Premises"), as legally described in Exhibit A attached hereto; and

WHEREAS, the Premises consists of an approximately 13,300 square foot lot, improved with a single story building containing approximately 8,000 square feet of floor area; and
