Be It Ordained by the City Council of the City of Chicago:
SECTION 1. Permission and authority are hereby given and granted to OLAVI GOOS, upon the terms and subject to the conditions of this ordinance to construct, install, maintain and use one (1) Sewer(s) under the public right-of-way adjacent to its premises known as 1639 N. Hermitage Ave
Said Sewer(s) at N. Hermitage Avenue measure(s):
One (1) at six point five eight (6.58) feet in length, and point five (.5) feet in width for a total of three point two nine (3.29) square feet. Said sewer line shall have a six (6) inch inspection opening.
The location of said privilege shall be as shown on prints kept on file with the Department of Business Affairs and Consumer Protection and the Office of the City Clerk.
Said privilege shall be constructed in accordance with plans and specifications approved by the Department of Transportation (Office of Underground Coordination) and Department of Water Management.'
This grant of privilege in the public way shall be subject to the provisions of Section 10-28-015 and all other required provisions of the Municipal Code of Chicago.
The grantee shall pay to the City of Chicago as compensation for the privilege #1124568 herein granted the sum of four hundred ($400.00) per annum in advance.
A 25% penalty will be added for payments received after due date.
The permit holder agrees to hold the City of Chicago harmless for any damage, relocation or replacement costs associated with damage, relocation or removal of private property caused by the City performing work in the public way.
Authority herein given and granted for a period of five (5) years from and after Date of Passage.

General Grant of Privilege Permit Application
The scope of work
I need a permission to install 6" inspection opening in the City sewer line outside of my property lines It should situate on the green area between the walkway and street because our house is built directly attached with the walkway (see the picture) Address 1639 North Hermitage Ave, Chicago, IL 60622
The reason for this work is root blockage in sewer line from our house to City sewers The blockage caused very bad flooding in our house, which was very costly, over $7,000 The blockage and its position was checked with camera system The sewer line was first cleaned with 2" rooter but it was not efficient in 6" pipe The second time it was cleaned with high pressure jet cleaner There is still some root growth in pipe, which is needed to get out and there is a danger that the roots will grow and block the sewer line again To keep the sewer line fully open it requires a 6" cleanout opening, which is City's responsibility to make according to the private dram program
We have had City's inspector to see the situation with camera system by a private plumber constructor in the beginning of June 2016
We are planning to move back to the house in 2-3 years
