SUBSTITUTE
ORDINANCE
BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF CHICAGO:
SECTION 1: On behalf of the City of Chicago as Landlord, the Commissioner of the Department of Fleet and Facility Management is authorized to execute a Lease agreement with Public Health Institute of Metropolitan Chicago, as Tenant, to permit the occupancy of 1,308 square feet of clinical office space located at 641 West 63rd Street for the provision of psychological and counseling services; such Lease to be approved by the Commissioner of the Department of Public Health and approved as to form and legality by the Corporation Counsel in substantially the following form:

THIS LEASE is made and entered into this day of , 2017,
("Commencement Date") by and between the CITY OF CHICAGO, an Illinois Municipal Corporation and Home Rule Unit of Government (hereinafter referred to as "Landlord" or "City") and PUBLIC HEALTH INSTITUTE OF METROPOLITAN CHICAGO, an Illinois nonprofit corporation (hereinafter referred to as "Tenant").
RECITALS
WHEREAS, Landlord is the owner of the real property commonly known as 641 West 63rd Street, Chicago, Cook County, Illinois, which is improved with a one story building with basement ("Building") used as the Englewood Neighborhood Health Clinic; and
WHEREAS, Tenant wishes to provide psychological counseling, group counseling, family therapy, substance abuse counseling, and specialized trauma services at the Englewood Neighborhood Health Clinic; and
WHEREAS, Landlord has agreed to lease to Tenant, and Tenant has agreed to lease from Landlord, approximately 1,308 square feet of clinical office space in the Building.
NOW THEREFORE, in consideration of the covenants, terms, and conditions set forth herein, the parties agree and covenant as follows:
