BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF CHICAGO.


SECTION 1: On behalf of the City of Chicago as Landlord, the Commissioner of the Department of Fleet and Facility Management is authorized to execute a Lease with Healthcare Alternative Systems, Inc., as Tenant, for use 89 square feet of office space at the Lower West Side Neighborhood Health Center, located at 1713 South Ashland Avenue, for the provision of behavioral health services; such Lease to be approved by the Commissioner of the Department of Public Health and as to form and legality by the Corporation Counsel in substantially the following form:

THIS LEASE (as may be amended from lime to time, the "Lease") is made and entered
into this day of , 2017, ("Commencement Date") by and
between the CITY OF CHICAGO, an Illinois Municipal Corporation and Home Rule Unit of Government (hereinafter referred to as "Landlord" or "City"), acting by and through its Department of Fleet and Facility Management and its Department of Health, having its principal offices at Cily Hall, 121 North LaSalle Street, Chicago, Illinois 60602. and HEALTHCARE ALTERNATIVE SYSTEMS, INC., an Illinois nonprofit corporation (hereinafter referred to as "Tenant"), having its principal offices at 2755 West Armitage Avenue, Chicago, Illinois 60647.
