ORDINANCE
NORTHWESTERN MEMORIAL HOSPITAL Acct. No 85392 - 7 Permit No. 1147307

So It Ordained by the City Council of the City of Chicago:
SECTION 1. Permission and authority are hereby given and granted to NORTHWESTERN MEMORIAL HOSPITAL, upon the terms and subject to the conditions of this ordinance to maintain and use, as now constructed, three (3) Conduit(s) under the public right-of-way adjacent to its premises known as 710 N. Fairbanks Ct
Said Conduit(s) at N. Fairbanks measure(s):
One (1) at four hundred seventy-six (476) feet in length, and point one (.1) feet in width for a total of forty-seven point six (47.6) square feet.
Said Conduit(s) at N. Superior measure(s):
One (1) at five hundred sixty-three (563) feet in length, and point one (.1) feet in width for a total of fifty-six point three (56.3) square feet.
Said Conduit(s) at N. St Claire measure(s):
One (1) at two hundred fifty-five (255) feet in length, and point one (.1) feet in width for a total of twenty-five point five (25.5) square feet.
The location of said privilege shall be as shown on prints kept on file with the Department of Business Affairs and Consumer Protection and the Office of the City Clerk.
Said privilege shall be constructed in accordance with plans and specifications approved by the Department of Transportation (Division of Project Development) and Department of Transportation (Office of Underground Coordination).
This grant of privilege in the public way shall be subject to the provisions of Section 10-28-015 and all other required provisions of the Municipal Code of Chicago.
The grantee shall pay to the City of Chicago as compensation forthe privilege #1147307 herein granted the sum of seven thousand seven hundred sixty-four ($7,764.00) per annum in advance.
A 25% penalty will be added for payments received after due date.
