SECTION 1. Pormiesion and authority are hereby given and granted to PRESENCE MEDICAL OFFICE BUILDING, upon the terms and subject to the conditions ol this ordinance to maintain and use two (2) sign(&) projecting over the pubGc right-of-way attached to its premises known as 30<18 W. Polerson Avie

The location ot said privilege shall bo as shown on prints kepi on tele with the Department of Business Affairs and Consumer Protection and the Office of lha City Clerk.
Said privilege shall bo constructed in accordance with plans and specifications approved by mo Department of Transportation (Division of Project Development) and Zoning Department - Signs.
This grant of privilege in lha public way shall be subject to the provisions of Section 10-28-015 and aO other required, provisions of the Municipal Code of Chicago.
The o/entee shall pay to Ihe City of Chicago as compensation for the privilege #1147862 herein granted the sum of six hundred ($600.00) per artnuni in advance.,
A 25% penalty wiB be added for payments received after due date.
The permit holder agroe3 to hold ihe City of Chicago harmless for any damage, relocation or replaoomont costs associated with darnago. rotecalion or removal of private property caused by the Cily performing work in the public way.;
