At the request ofthe Commissioner of Assets, Information and Services. 1 transmit herewith an ordinance authorizing the execution of a lease agreement with the Public Health Institute of Metropolitan Chicago.

ORDINANCE
BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF CHICAGO:
SECTION 1; On behalf of the City of Chicago, as Landlord, the Commissioner of the Department of Assets, Information & Services is authorized to execute a Lease Agreement with Public Health Institute of Metropolitan Chicago, as Tenant, to permit the occupancy of approximately 9,600 square feet of space located at 6337 South Woodlawn Avenue for the provision of psychological and counseling services; such Lease to be approved as to form and legality by the Corporation Counsel in substantially the following form:

LEASE AGREEMENT
THIS LEASE (the "Lease") is made and entered into this day of ,
2021 (the "Commencement Date") by and between, the CITY OF CHICAGO, an Illinois municipal corporation and home rule unit of government, acting by and through its Department of Assets, Information & Services ("Department"), and having its principal offices at 121 N. LaSalle Street, Chicago, IL 60602, ("Citv") and PUBLIC HEALTH INSTITUTE OF METROPOLITAN CHICAGO, an Illinois non-profit corporation (hereinafter referred to as "Tenant").
RECITALS
WHEREAS, the City is the owner of the real property located at 6337 South Woodlawn Avenue, Chicago, Cook County, Illinois, 60637 (the "Property"); and
WHEREAS, the Property is improved with a building utilized as the Woodlawn Neighborhood Health Center, comprised of approximately 13.968 square feet of space (the "Building,"); and
