BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF CHICAGO-


SECTION 1: On behalf of the City of Chicago as Landlord, the Commissioner of the Department of Assets, Information & Services is authorized to execute a Lease Agreement with Westside Health Authority, as Tenant, for use of vacant land located at 4453 West Madison Street; such agreement to be approved by the Department of Planning & Development and as to form and legality by the Corporation Counsel in substantially the following form:

LEASE AGREEMENT

THIS LEASE AGREEMENT (the "Lease") is made and entered into this day of
, 2022 (the "Commencement Date") by and between, the CITV OF CHICAGO,
an Illinois municipal corporation and home rule unit of government (hereinafter referred to as "Landlord" or "City"), and WESTSIDE HEALTH AUTHORITY, an Illinois not-for-profit corporation ("Tenant").

RECITALS

WHEREAS, the City is the owner of real property located at 4453 West Madison Street, Chicago, Cook County, Illinois (PIN 16-15-10-003-0000), consisting of vacant land that is not utilized for municipal operations; and

WHEREAS, Tenant is nonprofit organization that supports the local community through various programs to help improve the health and well-being of the community; and

WHEREAS, Tenant seeks use of the Premises (as hereinafter denned) for the establishment of an outdoor community plaza; and

WHEREAS, the City has agreed to o lease to Tenant, and Tenant has agreed to lease from the City, the Premises, upon the terms and conditions set forth herein....
